If a student is modified for a certain subject:
1. check the IEP box for that subject only
2. State at the beginning of the Strengths/Next Steps for Improvement comment section: “This grade is based upon the achievement of expectations in the IEP that vary from the grade ____ (enter the grade here) expectations and are an increase/decrease (pick one of these words) in the number and/or complexity of the curriculum expectations.
3. Enter your comments which should directly match up with the IEP expectations.
If a student has an assistive technology alternative page:
1. Your evaluation should be in the third column on the student’s IEP. Please include reference to how the student is able to use the technology in the specific subject areas on the report card (i.e., through the use of Google Docs for each stage of the writing process……).
If a student has an alternative page for any of the learning skills and work habits(i.e., organization, self-regulation) :
1. State in the Learning Skills and Work Habits Strengths/Next Steps for improvement that the student has IEP expectations for ____(indicate the name of the IEP alternative page here i.e., self-regulation) and indicate what the student is able to demonstrate.
2. Your evaluation should be more detailed in the third column on the student’s IEP.
If a student has an alternative page for anything else (i.e. self-advocacy):
1. Your evaluation should be in the third column on the student’s IEP and next steps need to be included.
Comments for academic subjects are written directly on the report card.
Comments for alternative subjects are written on the Alternative page.
Like the academic subjects, next steps must be included for comments on the alternative pages as well.

Your evaluation on the Learning Skills is based on what you have assessed up until you have started your report cards.

"Student name" joined our class on "first date at our school". This evaluation is based on assessments since this date. "Student name" has adapted...to his/her new school environment.
